Pre-Participation Physicals Registration for 2026-2027

In an effort to help prepare for the 2026-2027 school year, Hendricks Regional Health has created a schedule to provide high-quality, low-cost pre-participation physical exams. 

Who

2026-2027 middle school and high school students from Hendricks Regional Health-partnering schools needing a pre-participation physical.

When/Where

• Sunday, April 19th at HRH Danville Hospital – 1000 E Main St, Danville {Enter through East Entrance} 
• Sunday, May 3rd at Avon YMCA – 301 Satori Pkwy, Avon {Enter Door 15 on the East side of the building}
• Thursday, May 7th at Ben Davis High School – 1200 N Girls School Road, Indianapolis {Enter Door 5} FOR WAYNE TOWNSHIP STUDENTS ONLY
• Sunday, May 17th at Hendricks Regional Health Greencastle Medical Center – 1033 Indianapolis Road, Greencastle {Enter through main entrance} 
• Sunday, May 17th at North Montgomery High School {Enter Door 17}

$30.00 A portion of the fee goes back to the schools.

What to Bring

• Physical Form — Completed printed paper copy of the IHSAA Pre-Participation History Form
• Glasses/Contacts for vision testing (if necessary)
• QR code from confirmation email

Questions/Information

• Appointment Only: No walk-ups will be permitted.
• Time Commitment: The complete physical exam will take approximately one hour.
• Venue Information: The locations are only for use of the space; onsite staff will not be able to answer specific questions regarding the event.
• Alternative Options: If you cannot attend on the dates listed, HRH Immediate Care Centers offer pre-participation physicals for a $52 flat fee. Alternatively, you may schedule an appointment with your primary care physician.
• School Systems: For assistance with Final Forms or other school-based systems, please contact your school district directly.

Cancellation/Refund Policy

Cancellations must be completed 24 hours in advance . Requests for a refund due to extenuating circumstances must be in writing via email to sportsphysicals@hendricks.org.
