Important Tax Documentation Notice: How To Access Your ACA Form 1095-C

If you were employed by Hendricks Regional Health in 2025, your 2025 ACA Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) is now available for download in Workday. The instructions are below. You may also request a paper copy from Human Resources. The team's contact information is below.

Workday Download Instructions

1. Once in Workday, locate the search field at the top of the screen
2. In the search field, type My ACA Forms and click the My ACA Forms report that displays
3. Click View/Print in the 2025 line
