Patients who are managing complex or chronic medical conditions and challenging economic or social conditions often need support in achieving optimal health. This is where Hendricks Regional Health's Population Health team can help. The team pioneers and expands innovative new models of care that serve patients at high medical risk.

Expanding Access

Our innovative programs expand access to care, improve patient outcomes, and reduce avoidable healthcare costs, helping patients stay healthy outside of the doctor's office.

Our team:

  • Addresses the social determinants of health.
  • Improves outpatient and primary care management of chronic conditions.
  • Improves care coordination, risk-stratification, and outcomes tracking across providers and systems.
  • Partners closely with our community to ensure that our community’s needs inform all of our programs.

Chronic Care Management (CCM)

CCM is a Medicare-covered service that connects you with a dedicated healthcare team to support you between doctor visits. With personalized care, CCM can reduce stress, help you stay on track with your health goals and help improve your quality of life.

Do You Qualify?

You may be eligible if you:

  • Have Medicare (primary or secondary)
  • Live with 2 or more chronic conditions expected to last 12+ months
  • Have seen your provider in the last year
  • Are not enrolled in another care management program

Is There a Cost?

CCM is covered by Medicare. Depending on your plan, there may be a small monthly copay, but many patients pay little or nothing.

Colorectal Cancer

Colorectal cancer is the second leading cause of cancer death in the United States, and the National Cancer Institute estimates that 4.5% of men and women will be diagnosed with this life-threatening disease during their lifetimes. Routine colorectal cancer screenings can prevent up to 90% of colon cancer deaths.
