What Are We Measuring?

PSI‑08 (Patient Safety Indicator 08), developed by the Agency for Healthcare Research and Quality (AHRQ), tracks the rate of in‑hospital fall‑associated fractures per 1,000 patient discharges. While it originally focused on hip fractures, it now includes other fractures caused by falls during a hospital stay. This measure helps identify potential safety concerns and highlights opportunities to strengthen fall‑prevention practices and reduce patient harm.

This graph reflects the January data refresh on the CMS Care Compare website, covering the January 2024, 2025, and 2026 reporting cycles. A lower rate indicates stronger performance. Hendricks Regional Health has performed at the national rate across all three refresh cycles.

What Are We Doing to Prevent Falls with Injury?

Hendricks Regional Health has an interprofessional Falls Taskforce dedicated to reducing falls and supporting safe mobility for every patient. This team leads fall‑prevention initiatives, partners closely with clinical nurses, and engages patients by focusing on their individual mobility goals and overall quality of life. Our fall‑prevention strategies include:

  • Patient-mobilization training for associates
  • Universal fall-prevention measures
  • Use of the Morse Fall Risk Scale, with enhanced precautions for scores ≥ 50
  • Bed alarm
  • Chair alarm
  • Gait belt
  • Yellow light outside patient rooms to signal fall risk
  • iBed safety features
  • Call lights within easy reach
  • Assistive devices when appropriate
  • Ceiling lifts and lift-assist transport devices
  • Slide sheets
  • Grab bars
  • Inpatient hourly rounding

We also conduct regular audits to ensure all fall‑prevention measures are consistently in place for patients identified as high risk.
