What Are We Measuring?

The PSI‑90 (Patient Safety and Adverse Events Composite), developed by the Agency for Healthcare Research and Quality (AHRQ), is a measure that evaluates overall hospital safety by tracking rates of potentially preventable, hospital‑related adverse events. It combines several patient safety indicators — including infections, falls, surgical complications, and other injuries — into a single weighted score. The Centers for Medicare & Medicaid Services (CMS) uses this measure in key programs that link safety performance with financial accountability, helping ensure hospitals maintain strong, patient‑centered safety standards.

What Are We Doing to Improve PSI‑90 Performance?

Improving PSI‑90 performance requires a coordinated, organization‑wide approach grounded in evidence‑based practices and accurate clinical documentation. Our strategies include:

  • Implementing proven protocols to prevent falls, pressure ulcers, infections, and other safety events
  • Providing training and support for our Clinical Documentation Improvement (CDI) teams
  • Using the electronic health record (EHR) as a shared platform to enhance communication and offer real‑time clinical decision support
  • Monitoring our performance closely and benchmarking against CMS national rates
  • Reviewing every PSI‑90 event to identify opportunities for ongoing improvement
