Hendricks Regional Health applies a person-centric approach to skin care, encompassing all ages and stages, promoting the highest level of pressure injury prevention for all patients. To ensure equitable health care when assessing skin, skin tone diversity is highlighted. Acknowledging skin tone variation allows for recognition and identification of at-risk skin and early skin breakdown. Patient skin is assessed daily and evaluated using different risk assessment tools depending on age, adult patient (Braden Scale) and pediatric patient (Braden QD). Tailored pressure injury prevention interventions based on individual patient needs are implemented and consistently reassessed during a hospital admission.
