Rotation Evaluation

Thank you preceptors for taking time out of your busy schedules to educate our future doctors. We have tried to simplify the evaluation process for you as much as possible. You have several options to submit these for your convenience. You can electronically submit your evaluation using the generic link shown below. You can also print a paper copy by downloading it with the provided link. Another option would be to free text your thoughts by emailing them to mschul2@hendricks.org so that I can complete the form and submit it to Marian. 

Students, if your preceptor prefers that you provide them a written copy of your evaluation, you can print the form by downloading it from this page.
