Five-Step Model of Care

We utilize a five-step model of care to improve the health of your employees:

Step 1: Our registered nurses certified in wellness coaching reach out to make personal contact with the employee. By talking about medical history, concerns and other relevant issues, we together lay the groundwork for realistic, achievable wellness goals.

Step 2: We work with the employee’s physician to ensure goals coordinate and support the current care plan.

Step 3: Our staff reviews relevant medical documentation, determines what services have already been provided (eliminating repetition) and monitors compliance with physician recommendations.

Step 4: We connect with other providers, services and resources such as diabetes education, physical therapy evaluation and nutrition counseling to prevent  further medical issues or complications. Also, we may partner with one of our knowledgable pharmacists to conduct a thorough medication review to identify cost-saving opportunties.

Step 5:
Our wellness nurses help stabilize the employee’s current medical condition when possible, and develop additional prevention strategies for other chronic diseases. The goal of this final step is to transition the participant back to an improved state of health, which will be effective in lowering medical costs for both employee and employer.
