Your medical records can include a variety of information about your health history, such as medical test results and discharge instructions from your stay. For your convenience, we offer our patients the choice to receive their records on paper or electronically through a secure delivery server.
To obtain copies of radiology (x-ray) images, please contact the Hendricks Regional Health Radiology Department at (317) 745-3425 and they can assist you by providing a disc with your images.
THREE STEPS TO REQUEST YOUR MEDICAL RECORDS
1. DOWNLOAD an Authorization for the Use or Disclosure of Health Information Form
2. COMPLETE the form, being sure to DESIGNATE whether you would like to receive your medical records by paper (through the postal mail) or electronically (through a secure link that will be emailed to you). If you prefer to receive your records
electronically make certain you provide a valid email address.
3. SUBMIT the form by fax or mail to:

More About Receiving Your Records Electronically
Patients that request their records electronically will receive an email within three business days containing a link to access your records. Once you have clicked the link, you will be asked security questions to further safeguard your medical information and privacy. Then you can print and/or download your records to your personal files.
If you don't receive an email within three business days, check your spam or trash folder.
Tip: Those who request records electronically will receive an email from requests@discoveryhrs.com. By adding this address to your contacts list, you can help ensure your email program does not send it to your spam folder.
To request a change in your medical record, complete the Request for Amendment to Health Information Form and submit it to the above address.
