Medical Record Services


Our Medical Records office is here to assist you with requesting access to your medical records, or an amendment to your health information. Please review the available services below, and contact our team at (317) 745-8635 for questions or additional support.

Using MyChart to Access Your Medical Records



We are proud to offer a secure portal called MyChart that allows you to access your medical records online for free. You'll find MyChart to be a convenient way to track your Hendricks Regional Health appointments, communicate with your healthcare provider’s office, renew prescriptions and see your test results. 

You'll receive MyChart registration information as part of your doctor’s appointment visit summary or hospital discharge instructions. You can also request a MyChart account by visiting MYCHART.HENDRICKS.ORG and clicking on “Request an Access Code.”

Once you are logged into MyChart, click on the “Messaging” tab and then “Request Your Hospital or Doctor’s Office Record” to submit your request. You may also find test results, immunization records and medication information under the “Health” tab. 

We strive to respond to medical record requests within two business days. Once your request is processed, you will receive a notification to log into MyChart and retrieve your records.

Using MyChart to Access Another Medical Record
You can use MyChart to access the medical record of another family member (such as a child, parent, adult for whom you are legal guardian, spouse or partner) through a feature called “proxy.” Other accounts that you have access to will appear in your personal MyChart portal. You need to have a MyChart account to get proxy access to someone else’s account. Talk to the team at your healthcare provider’s office about how to be granted proxy access.

For Medical Records Not Available in MyChart

Depending on when you received care, it is possible that all of your records are not available through MyChart. In these situations, our Health Information Management team can assist you in other options to securely receive your medical records. Please use this form to request access to your health information not in MyChart.
