Just click on one of the card selections below. Fill out the form with the patient's name, your message, and your name. Click the Send button when you are finished. That's all there is to it!
Messages will be personally delivered by our ambassador staff during the following hours:
Monday through Friday: 8 a.m. to 8 p.m.
Saturday and Sunday: 10 a.m. to 8 p.m.
Messages will be physically handled by members of our staff; therefore, they should not contain any private or confidential material. We maintain the right not to deliver messages that contain inappropriate subject matter.
